Entry Application

Edit, Save, and Print PDF Application - Open the application form and fill in the requested information.  Once the form is completed, save the form to your computer to print for sending via postal mail or, simply attach the saved form to an email.

Open Application Checklist - This checklist is designed to assist you in the preparation of your entry application. Be sure to have all proper signatures to certify each area. 

Housing Request 

The following link will take you to the Lake County Convention and Visitors Bureau who will be coordinating the housing for the tournament again this year.  The process is the same as 2007.
  1. Within the link provided above the team contact will click on 2008 Lutheran Basketball Housing List to view the approved housing list.
  2. The team contact will fill out the housing request.
  3. Once completed the team contact will click submit and receive the following message: 
    Thank you for your submission.  Every effort will be made to accommodate your team’s housing request.  You will be contacted via e-mail before March 12th with your hotel assignment. 
  4. The South Shore Sports Promotions (Lake County CVB) will make the room assignments and email the team contact a username and password to access our housing bureau to block their rooms and confirm rooming lists.
  5. The hotel will receive the information and the teams are confirmed.
